Type specimen: USNM 51704, 51826 (cotypes). Its type locality is 54s - Near mouth of North side Two Mile Canyon, 2 miles se of Malad, Wasatch Mts, which is in a Menevian carbonate limestone in the Langston Formation of Idaho.
